As a new patient to Forest North Dental, we want to provide you with an overview of what to expect during your first visit to our office.
When you arrive for your appointment, our friendly staff will greet you, collect your new patient intake forms, and process your information. Our intake forms allow our dentists to get to know you and your individual needs.
If you have missing or poorly aligned teeth, pain related to dental or other oral issues, or simple routine dental care needs, our reliable team has the appropriate solution for you. You can have your teeth whitened if they are not as bright as you would like them to be. If you have had an accident that damaged your teeth or have not been able to give them proper care, Dr. Rosenblatt or Dr. Singapori may be able to restore them with veneers. Implants are available if other procedures cannot save your natural teeth. If you have pain, swelling, or discomfort, our Dentists will consult with you to determine the best course of personalized treatment for your needs.
We offer care to all ages. From simple teeth cleanings and checkups to complex cosmetic procedures, we will be thorough, yet gentle, with our treatment approach. New Patient Forms
To prepare for your upcoming appointment, Forest North Dental provides our patient forms online. Arriving with completed forms before your visit will also help ensure timely check-in at your appointment time.
At Forest North Dental we accept most insurances; including payments from PPO and indemnity plans. We accept all PPO plans within Humana and Delta Dental. Insurance will help to defray some of the costs of treatment and we will help you by filing your insurance for you, immediately after treatment. We will also try to estimate what your insurance will cover for each procedure. You will be responsible for the patient portion at time of visit. We cannot guarantee reimbursement by an insurance company and you are ultimately responsible for any remaining balances not paid for by the insurance company.
At Forest North Dental, we accept all major credit cards, ATM cards, cash, and personal checks.
We also offer long-term payment plan options by partnering with Care Credit. You can get approved in less than 5 minutes.
